Job Description

Under the general direction of the Divisional Director, the Employment Success Manager (ESM) provides and coordinates supportive services, specific to work and training opportunities for all consumers served through Community Rebuilders programs. Services provided are necessary to assist eligible households to improve their economic security and overall housing stability. The ESM will work with consumers, who have been referred by Community Rebuilders Housing Resource Specialist who identify economic insecurity as a barrier, to obtaining or maintaining permanent housing. The ESM will also promote the awareness of the Community Housing Connect Employment Platform and be the main point of contact with community Work Force Development agencies to best meet the needs of Consumers.

Services to include but not limited to:

Creation of employment plans utilizing the Signify and Euzen connect platforms

Supporting consumers through the Euzen connect job training and match process

Interview coaching and guidance, in order to support, the overall goal of increasing income and self-sufficiency

Utilize Strengths Based Housing First Case Management Services when developing economic security and stability goals and outcomes

Work in conjunction with consumers, their HRS’s and established housing plans

Assistance with activities that promote the awareness of the Community Housing Connect Employment platform and G.R.A.C.E. Network with regards to the employment pathway.

Protect the confidentiality of all persons served in compliance with organizational policy and federal requirements and regulation.

Key Responsibilities:

Interact with consumers in a manner consistent with the mission and vision of Community Rebuilders

Facilitate and coordinate support service activities for participants to promote achievement of goals and objectives, including development of an economic success and stability plan.

Utilize the G.R.A.C.E. Network and Signify platform. Accept and receive referrals, follow employment workflow, track employment goals and progress.

Utilize the Euzen connect platform to schedule meetings and support the consumer job and training matching process.

Ensure consumers are responded to within 24 hours.

Community Involvement - Build relationships with workforce development and social service resource groups to join the platform and ensure their resources are listed in our resource library and they cross-refer to us.

Configure technology to ensure intake and follow-up processes are best meeting consumer needs. This includes improvement of Signify and Euzen connect platform design and set up for optimal results.

Monitor and track metrics to ensure program employment goals and performance measurements are being met.

Maintain client files per program guidelines and funding requirements. Collect and report program data, per funding requirements.

Provide coaching to consumers to support the goals of increased income and self-sufficiency.

Attend team meetings, staff meetings, and training workshops.

Contribute to the team in service delivery outcome achievements.
